The HOPE team provides MHSA Full Service Partnership (FSP). This includes intensive case management and rehabilitation services to adults with serious mental health conditions or co-occurring mental health and substance use disorders. Participants in the HOPE program receive intensive support towards recovery goals and are encouraged to fully participate in Wellness and Recovery Center (SYBH).  The goal of this program is to help participants reach and maintain stability, participate fully in community life, decrease isolation, increase independence and support a sense of belonging. Services are provided based upon participants individual wellness and recovery goals. 

Services can include case management, living arrangement assistance, facility placement, individual and group rehab services, collateral and peer support programs, and access to employment and housing resource specialist. 

Intervention counselors are available to clients on a 24/7 basis
