Hmong Translation 

The Hmong Outreach Center serves bilingual and Hmong-speaking only adults and families delivering culturally and linguistically appropriate services. The Hmong center outpatient behavioral health program was designed to provide a full range of coordinated therapeutic and support services in the form of triages, intake assessments, treatment planning, diagnosis and treatment of mental health conditions and co-occurring mental health and substance use disorders. The Hmong Center provides bilingual/bicultural staff, who are trained interpreters, who can provide services in the clients’ preferred language. 

Services offered include individual therapy, group and individual rehabilitation services, case management, linkage to other adult services such as medication support or substance use disorder treatment and linkage to community resources and supports. Further service linkage and coordination includes housing assistance, counseling and education on nutrition, primary health care, natural healers, spiritual leaders, and gardening. 

Services are accessed by calling the Hmong Outreach Center directly to request services, or by attending the Adult Services Open Access Clinic held Monday-Wednesday 8 am–2 pm at 1965 Live Oak Blvd. Yuba City CA, 95991.
